Below, please find the new patient paperwork that needs to be filled out and brought with you along with your insurance information to your first appointment. Please plan to arrive 15 minutes early to ensure your paperwork is complete.
Upon entering our office, we will help you get settled. You will complete any additional paperwork, providing us with your health history and information. Please complete the following forms ahead of time. You have the choice of either printing them or filling them out online.

Getting Here
Traveling on Rte 2 East, exit 41 toward W. Acton. Bear right off the exit, approximately 1.7 miles down the road, take a left onto Arlington Street. Our office is the building with the teal awnings on the left. The parking lot can be accessed using the driveway right before the building.
Traveling on Rt 2 West, take exit 43 (left exit), onto Rte 111. In 2 miles, take a right onto Central Street. Then take your first right onto Arlington Street. Our office is the building with the teal awnings on the left. The parking lot can be accessed using the driveway right before the building.
What do I need before my exam?
During your first visit, please bring any recent X-rays, MRI’s, or Cat Scans that are related to any Spinal problems you may be suffering from. Please arrive to the exam fifteen minutes early so that any extra paperwork can be filled out. Make sure to bring your health insurance card and preferred payment method to your first appointment,
The doctor will take a detailed history, and perform several different objective tests to determine if you have any nerve system pressure that may be causing your secondary symptoms. These tests will include a balance analysis, digital structure analysis, surface electromyography, and a muscular and intersegmental palpatory scan of your spine. These tests are all very easy and completely painless.
After the exam, the doctor may also refer you to an off-site facility for any further imaging that needs to be done.
